CDPH — California Department of Public Health / Cal Health Find:
CDPH Cal Health Find — Facility Search
This is one of the most important sources for a California SNF case. Search the facility by name and you can obtain its licensing information, complaints, facility-reported incidents, deficiencies, enforcement actions and Class AA, A and B citations. CDPH states that Statements of Deficiencies/Plans of Correction (CMS-2567s) approved after March 23, 2011 are displayed on individual SNF pages.
CMS — Centers for Medicare & Medicaid Services / Medicare Care Compare:
CMS Medicare Care Compare — Nursing Homes
This is where I would check the facility's overall Five-Star rating, health-inspection rating, staffing rating, quality-measures rating, inspection deficiencies, staffing information and other federal data. CMS explains that Care Compare provides the 1–5 star ratings as well as health and safety inspection information.
CDSS — California Department of Social Services:
This is particularly important for an RCFE/assisted-living case, rather than an SNF. The database provides five years of facility information and includes licensing reports and complaint investigation reports. CDSS states that additional information and details regarding citations can be obtained from the applicable regional office.
CCLD — Community Care Licensing Division:
California Community Care Licensing Division
CCLD is within CDSS, so it isn't really a separate regulatory agency from CDSS. CCLD regulates community-care facilities, and its Senior Care Licensing Program licenses and monitors RCFEs. The CDSS Facility Search above is the public-facing database I would use to locate CCLD inspection reports, complaints and citations for a particular RCFE.
CANHR — California Advocates for Nursing Home Reform:
CANHR is not a government regulator. It is a California nonprofit advocacy organization, but its site is useful for nursing-home research, resident rights, care standards, complaints, regulations and links to government inspection resources. CANHR specifically directs users to CDPH's Cal Health Find and CMS Nursing Home Compare/Care Compare for facility inspection histories.
